Abstract (inglese)

Juvenile Localized Scleroderma (JLS) is a chronic inflammatory disease. Abnormalities in the Immune System (IS) and in the microvasculature are important. Forty two JLS patients and 39 healthy controls entered the study. They were analyzed at the beginning and then randomized into two subgroups, one treated with MTX and the other one treated with placebo. Lymphocyte distribution and NK cell receptor (NKR) expression was explored and EPCs were quantifyed in the peripheral blood of the patients by flow cytometric analysis and the inflammatory infiltrate in the skin lesions was evaluated by immunoistochemical analysis. JLS patients showed an enhanced expression of the NK cell activity, a decreased number of circulating EPCs and an infiltrate rich in T lymphocyte in the affected skin. MTX affected the disease locally, at skin level, by a consistent reduction of the inflammatory infiltrate while did not influence the immunological pattern in the peripheral blood.

Abstract (italiano)

La Sclerodermia Localizzata Giovanile è una malattia infiammatoria cronica. Sono state riscontrate anormalità del Sistema Immunitario e della microvascolatura. In questo studio sono stati arruolati 42 pazienti con diagnosi di JLS e 39 soggetti sani come controlli. I pazienti sono sati analizzati al momento dell'arruolamento e in seguito sono stati randomizzati in due gruppi, uno trattato con MTX e uno trattato con placebo. E' stata analizzata la distribuzione delle sottopopolazioni linfocitarie, l'espressione dei recettori delle cellule NK e il numero delle EPC nel sague periferico dei pazienti attraverso analisi citofluorimetrica; inoltre è stato caratterizzato l'infiltrato infiammatorio presente nelle lesioni cutanee mediante analisi immunoistochimica. I pazienti affetti da JLS studiati hanno mostrato un aumento della citotossicità delle cellule NK e una deplezione del numero delle EPC nel sangue periferico ed un infiltrato ricco di linfociti T a livello delle lesioni cutanee. Il MTX sembrava indurre una remissione dell'infiltrato infiammatorio cutaneo, ma non sembrava avere alcun effetto sull'assetto immunologico presente nel sangue periferico.
