SUMMARY A. OUTCOME OF PATIENTS PRESENTING THEMSELVES TO THE EMERGENCY DEPARTMENT WITH CHEST PAIN Background. Chest pain is one of the most common symptoms presented to the emergency department. It actually accounts for 5% - 8% of the total visits at the emergency room. Yellow triage tag is mainly assigned to patients with chest pain, when there are no alterations in their vital functions. ‘Short-stay observation unit’ is sometimes used for patients suffering from chest pain, which consists of additional monitoring in a specialised area of the emergency department, in order to exclude any case with high probability of acute coronary disease. The following table below shows the results of a retrospective study conducted at the Emergency Department of the “Ospedale dell’Angelo”, hospital located in the Venetian mainland of the Veneto region, in Italy. The study period was between 24th May 2010 and 23rd May 2011. The objective of the study was: -to investigate the outcome of patients with chest pain presenting to an emergency department; - to determine if triage is suitable for identifying chest pain with risk of acute coronary disease; -to evaluate if a ‘short-stay observation unit’ is useful and effective in case of undefined chest pain, that is potentially at risk of acute coronary disease. The results are as follows: 3,980 cases of chest pain were analyzed, which accounted for 4.71% of the 84,554 total visits to the emergency department during the period of the study. Among these, 2,449 patients (61.5%) were discharged, 714 (18.0%) were hospitalized, 4 (0.1%) died and 813 (20.4%) were transferred to short-stay intensive observation for further clinical bioumoral and electrocardiographic monitoring, and / or possible ergometric test. During primary triage, a red or yellow tag was assigned to 78.8% of cases of chest pain and a green or white tag was assigned to 21.2% of cases. The conclusive diagnosis revealed an acute coronary heart disease in 39.9% (285 patients) of the 714 patients who were directly admitted to the emergency room. Of the 813 patients submitted to short-stay observation unit, 73.4% (597 cases) were discharged, whereas 26.6% (216 cases) were admitted. Of 24.5% (199 cases) of the patients submitted to short-stay observation unit, 15.1% (30 cases) were discharged for chronic stable coronary disease and 84.9% (169 cases) were hospitalized for acute coronary syndrome. Finally, 454 patients with acute coronary disease were admitted to the emergency room and to short-stay observation unit, which accounted for 11.4% of the patients presenting themselves to the emergency department with chest pain. The death of four patients was due to complications of acute coronary disease. Furthermore, there was a prevalence of males over females among the patients admitted (64.4% males vs. 35.6% females). Triage proved to be sufficiently sensitive and specific in distinguishing the cases of chest pain for acute coronary disease at high risk from those at low risk. In fact, during primary triage the 78.8% of cases of chest pain were given a red or yellow tag (i.e., potentially life – threatening cases), and the remaining 21.2% of cases were given a green or white tag (therefore no life-threatening). The majority of patients who were given a green or white tag was sent home (84.3% vs. 55.4% of patients with a red or yellow tag), whereas a prevalent percentage of patients who were given a red or yellow tag was hospitalized (21%) or led to short-stay observation unit for further monitoring (23.5%). Patients used to stay in short-stay observation units slightly over 23 hours (23 hours and 32 minutes). Short-stay observation of chest pain at risk of acute coronary disease - in both patients with a yellow tag (high risk) and those with a green or white tag (low or intermediate risk) - turned out to be useful in reducing 'inappropriate' hospitalizations for nonspecific chest pain. In many cases it also allowed a rapid exclusion of acute coronary disease by means of telemetry monitoring, transthoracic echocardiography and ergometric tests, as well as cardiology specialist assessment. Conclusions Despite being a frequent symptom of presentation to the emergency department, acute chest pain and the risks attached require rapid clinical diagnosis and sometimes a diversified approach. If the cause of chest pain is not correctly identified, it could lead to fatal effects on the outcome of patients. Therefore, the intensive short-stay observation unit corresponds to the Chest Pain Unit, where patients with chest pain are further 'examined' to come to a definitive diagnosis, optimizing therapeutic - diagnostic treatments and the available resources. B. ANALYSIS OF IN-HOSPITAL DEATHS IN AN EMERGENCY DEPARTMENT Background. Dealing with critical health and life-threatening conditions is one of the main functions of the emergency department. During the initial assessment of the critically ill patient, assessment and stabilization processes have to be carried out in a very short period of time by following a step by step system, according to a scale of priorities (ABCD); shared and encoded diagnostic and therapeutic workups are often used to assess and stabilize the patient. Nevertheless, some life-threatening situations result in death. This is a retrospective study, conducted at the Emergency Department of the “Ospedale dell’Angelo” located in Mestre, in the north east of Italy. The purpose of the study was: - To analyze the most common causes of death in patients presenting themselves to an emergency department; - To evaluate the autopsy rate among in-hospital deaths, defined as the ratio between the number of autopsies carried out and the total number of deaths examined; - To evaluate the accuracy of clinical diagnosis made by an emergency department at the time of death, taking as a reference standard the autopsy diagnosis. In-hospital deaths were examined between June 2008 and December 2011. Results During the period between June 2008 and December 2011, the total admissions to the emergency room in Mestre amounted to 284,827 cases. Patients who died during this period amounted to 223 persons, representing 0.078% of the total admissions. They presented the following characteristics: 28 cases in 2008, 14 males and 14 females, 50 cases in 2009, 24 males and 26 females; 83 cases in 2010, 41 males and 41 females, 62 cases in 2011, 27 males and 35 females. During the period taken into account (2008-2011), patients who died were 116 females and 107 males, who accounted for, respectively, 52.02% and 47.98% of in-hospital deaths. The average age of females was 83.42 ± 16.76 years ds (cases between 33 and 106 years), whereas the average age of males was 73 years ± 3.53 ds (cases between 22 and 96 years). Among the causes of death, heart, pulmonary and cardiopulmonary diseases represented more than 1/3 of in-hospital deaths both for males and females. Of 223 deaths, an autopsy was required in 12.55% of cases, equal to 28 patients (13 males and 15 females; the average age was, respectively, 59 and 70.2 years). Of 28 autopsy reports, only 23 cases were analyzed, accounting for 82.14%. In evaluating the correlation between the ‘discharge’ diagnosis and the ‘autopsy’ diagnosis, the 23 cases were divided as follows: in 16 cases (69.57%) there was no correlation, whereas in 7 cases (30.43%), the correlation was positive since both diagnoses were equivalent. In the analysis of in-hospital deaths, the average age of death for males was significantly lower than for females (59 vs. 70.26 years). The 'nonspecific' diagnoses related to cardiac arrest and / or cardio respiratory arrest, among others, accounted for 29.2% of cases in males and 37.07% of cases in females. In addition, the most frequent causes of death in females were those related to sepsis and multi-organ dysfunction. The causes of death due to traumatic events, especially road accident traumas, were more frequent in males than in females (15 vs. 3 cases). The analysis of the causes of death in patients undergoing autopsy showed that the totality of cases suffered from severe life-threatening diseases, that rapidly led to death. The most common diseases were heart and vascular diseases, in particular abdominal and brain vascular diseases. Conclusions Innovative therapeutic and instrumental tools do not always contribute to determine the true cause of death in patients. At times, the clinical diagnosis made by physicians is not accurate and it does not reflect the post-mortem examination. The anatomo-pathological assessment is not only extremely useful in medical practice in order to determine the cause of death, but it is also a source of knowledge to improve therapeutic-diagnostic workups, the outcome of the patients, as well as the clinical practice of physicians.

RIASSUNTO Di seguito, si sintetizza lo studio sull’esito dei pazienti che accedono al dipartimento di emergenza per dolore toracico (A) e una breve analisi dei decessi intraospedalieri (B) occorsi nel Pronto Soccorso dell’Ospedale dell’Angelo di Mestre, sito nella provincia veneziana del Veneto. A. OUTCOME DEI PAZIENTI CHE ACCEDONO AL DIPARTIMENTO DI EMERGENZA PER DOLORE TORACICO DELL’OSPEDALE DELL’ANGELO DI MESTRE Introduzione. Il dolore toracico è frequente motivo di accesso ad un dipartimento di emergenza. Rappresenta circa il 5-8% degli ingressi in un Pronto Soccorso (PS). In relazione al metodo di triage, il dolore toracico viene prevalentemente associato al codice cromatico giallo in assenza di alterazioni delle funzioni vitali. Talora, per escludere i casi ad alta probabilità di patologia coronarica acuta tra i pazienti affetto da dolore toracico, si ricorre all’Osservazione Breve Intensiva (OBI) che consiste in un ulteriore monitoraggio clinico strumentale del malato in un’area dedicata del dipartimento di emergenza. Si riporta, di seguito, i risultati di uno studio retrospettivo condotto presso l’Unità Operativa del Pronto Soccorso dell’Ospedale dell’Angelo, situato nella terraferma veneziana della regione Veneto. Il periodo di studio era compreso tra il 24 maggio 2010 e il 23 maggio 2011. Lo scopo dello studio era: -.verificare l’outcome dei pazienti affetti da dolore toracico che accedevano ad un dipartimento di emergenza; - analizzare l’appropriatezza del metodo di triage nell’individuare il dolore toracico a rischio di patologia coronarica acuta; -.valutare l’utilità e l’efficacia dell’”Unità dell’Osservazione Breve” (OBI) nei casi dolore toracico a genesi indefinita, ma a potenziale rischio di patologia coronarica acuta. I risultati sono i seguenti: Sono stati analizzati 3980 casi di dolore toracico, che rappresentavano il 4,71% degli 84.554 accessi totali al dipartimento di emergenza nell’anno considerato. Di questi, 2449 (61,5%) pazienti sono stati inviati a domicilio, 714 (18,0%) sono stati ospedalizzati, 4 sono deceduti (0,1%) e 813 casi (20,4%) trasferiti in OBI per ulteriore monitoraggio clinico, bioumorale ed elettrocardiografico e/o eventuale test ergometrico. Dei 714 pazienti, direttamente ricoverati dal pronto soccorso, la diagnosi conclusiva ha permesso di rilevare, nel 39,9% dei casi (285 pazienti), una patologia cardiaca coronarica acuta. I 4 pazienti deceduti erano evoluti in exitus per complicanze legate alla patologia coronarica acuta. Degli 813 pazienti transitati in OBI, il 73,4% veniva dimesso (597 casi), mentre il 26,6% veniva ricoverato (216 casi). Nel 24,5% dei casi transitati in OBI (199 casi), il 15,1% (30 casi) veniva dimesso per patologia coronarica cronica e stabile mentre l’84,9% (169 casi) veniva ricoverato per sindrome coronarica acuta. In definitiva, venivano ricoverati, dal Pronto Soccorso e dall’OBI, 454 pazienti affetti da patologia coronarica acuta che rappresentavano l’11,4 % dei pazienti che erano giunti al Pronto Soccorso per dolore toracico. Inoltre, tra i pazienti ricoverati vi è una prevalenza dei maschi rispetto alle femmine (maschi 64,4% vs 35,6% femmine). Il metodo di triage si è dimostrato sufficientemente sensibile e specifico nel distinguere i casi di dolore toracico ad alto rischio per patologia coronarica acuta da quelli a basso rischio. Infatti, al triage d’ingresso è stato attribuito il codice rosso o giallo (casi potenzialmente life – threatening) nel 78,8% dei casi di dolore toracico, mentre il codice verde o bianco (casi non ‘life –threatening’) nel rimanente 21,2 % dei pazienti. Per i casi codificati con codice cromatico verde o bianco vi era una prevalenza di invio a domicilio (84,3% vs il 55,4% dei pazienti con codice cromatico rosso o giallo), mentre per quelli codice cromatico rosso o giallo vi era una percentuale relativamente prevalente di pazienti ricoverati (21%) o avviati ad ulteriore monitoraggio in OBI (23,5%). La durata media di permanenza in OBI era di poco superiore alle 23 ore ( 23 ore e 32 minuti). Per i casi di dolore toracico ad alta probabilità per patologia coronarica acuta tra i soggetti con codice cromatico giallo e tra quelli con codice cromatico verde o bianco a rischio intermedio o basso, ‘l’Osservazione Breve Intensiva’ si e’ dimostrata utile nel ridurre i ricoveri ‘inappropriati’ per dolore toracico aspecifico e nel consentire una rapida esclusione della patologia coronarica acuta mediante il ricorso al monitoraggio telemetrico, all’ecocardiografia transtoracica e al test ergometrico nonché alla valutazione specialistica cardiologica. Conclusioni Pur rimanendo frequente motivo di accesso in Pronto Soccorso, il dolore toracico acuto e il rischio ad esso correlato richiedono un rapido inquadramento clinico e talora un approccio diversificato poichè il misconoscimento della patologia ad essa sottesa può produrre effetti fatali sull’outcome dei malati. In tal senso, l’osservazione breve intensiva può ricalcare il modello della Chest Pain Unit presente in altre realtà, ove i pazienti con dolore toracico vengono ulteriormente ‘osservati’ per giungere, in breve tempo, ad una diagnosi definitiva ottimizzando i percorsi terapeutico- diagnostici e le risorse a disposizione. B. ANALISI DEI DECESSI INTRAOSPEDALIERI DI UN DIPARTIMENTO D’EMERGENZA-URGENZA Introduzione. Tra le funzioni principali del dipartimento di emergenza vi è quella di fronteggiare le condizioni sanitarie di criticità e di rischio per la vita. Nella valutazione iniziale del malato critico è necessario attivare, quanto prima, i processi di valutazione e stabilizzazione, seguendo un percorso a gradini (step by step) ove l’ordine è rigidamente preordinato secondo una scala di priorità (A= airway; B= breathing; C= circulation; D= disability).Pur tuttavia, in alcune situazioni ‘life threatening’, l’evoluzione successiva è il decesso. Trattasi di uno studio di tipo retrospettivo condotto presso l’Unità Operativa del Pronto Soccorso dell’Ospedale dell’Angelo sito in Mestre, nel nord est dell’Italia. Scopo dello studio era: - analizzare le cause di decesso di più comune riscontro nei pazienti che afferiscono ad un dipartimento di emergenza; - dei decessi intraospedalieri, valutare il tasso autoptico definito come il rapporto tra il numero di autopsie effettuate sul numero totale dei decessi esaminati; - valutare l’accuratezza della diagnosi clinica formulata in un dipartimento di emergenza al momento del decesso, assumendo come standard di riferimento la diagnosi autoptica. Risultati I decessi intraospedalieri esaminati si riferiscono al periodo compreso tra giugno 2008 e dicembre 201l. Nello stesso periodo, gli accessi totali al Pronto Soccorso di Mestre ammontavano a 284.827 casi. I pazienti deceduti ammontavano a 223 soggetti e rappresentavano lo 0.078% degli accessi totali. Essi erano caratterizzati come segue: 28 casi nel 2008, 14 maschi e 14 femmine; 50 casi nel 2009, 24 maschi e 26 femmine; 83 casi nel 2010, 41 maschi e 41 femmine; 62 casi nel 2011, 27 maschi e 35 femmine. Nel periodo considerato (2008-2011), i pazienti deceduti erano 116 femmine e 107 maschi. L’età media era di 83,42 anni ±16,76 ds (casi di età compresa tra i 33 e i 106 anni) nelle femmine, mentre nei maschi era di 73 anni ± 3,53 ds (casi di età compresa tra i 22 e i 96 anni) e rappresentavano, rispettivamente, il 52,02% e il 47,98% dei decessi intraospedalieri. Tra le cause di decesso, quelle riconducili a patologia cardiaca, polmonare e cardiopolmonare rappresentavano più di 1/3 dei decessi intraospedalieri sia nel gruppo dei maschi che nel gruppo delle femmine. Dei 223 decessi, l’esame autoptico veniva richiesto nel 12,55% dei casi, ossia, in 28 pazienti (13 maschi e 15 femmine; l’età media era, rispettivamente, di 59 e di 70,2 anni). Dei 28 referti autoptici venivano analizzati solo 23 casi, ossia l’82,14% . Nella valutazione della concordanza tra la diagnosi di ‘dimissione’ e quella ‘autoptica’, i 23 casi erano così ripartiti: in 16 casi (69,57%) non vi era concordanza, mentre in 7 casi (30,43%) la concordanza era positiva cioè le due diagnosi erano uniformi e concordanti. Nell’analisi dei decessi intraospedalieri, l’età media dei maschi era significativamente inferiore rispetto alle femmine (59 vs 70,26 anni). Tra le cause di decesso, quelle per patologia cardiaca, polmonare e cardiopolmonare rappresentavano più di 1/3 dei decessi intraospedalieri sia nei maschi che nelle femmine. Le diagnosi ‘aspecifiche’ riconducibili ad arresto cardiocircolatorio e/o cardiorespiratorio e ad altro (exitus e cachessia) rappresentavano il 29,2% dei casi nei maschi e il 37,07% nelle femmine. Inoltre, in queste ultime, erano frequenti le cause riconducibili a sepsi ed a disfunzione multi organo. Le cause di decesso per eventi traumatici, in particolare, i traumatismi della strada erano più frequenti nei maschi rispetto alle femmine (15 vs 3 casi). Nell’analisi delle cause di decesso dei pazienti sottoposti ad autopsia, la totalità dei casi soffrivano di patologie severe, ‘life threatening’, a rapida evoluzione in exitus. Tra queste, erano prevalenti le patologie cardiache e vascolari sia addominali che cerebrali. Conclusioni Pur disponendo di ausili terapeutici e strumentali innovativi, questi non sempre contribuiscono a definire la vera causa di morte dei pazienti. Talora, la diagnosi clinica formulata dai sanitari è approssimativa e non corrispondente all’esame autoptico. L’accertamento anatomo-patologico rimane ancora di grande utilità nella pratica medica per stabilire la causa di morte, ma anche come fonte di conoscenza per migliorare i percorsi terapeutico- diagnostici, l’outcome dei malati nonché la pratica clinica dei sanitari.

Scopo dello studio era: - analizzare le cause di decesso di più comune riscontro nei pazienti che afferiscono ad un dipartimento di emergenza; - dei decessi intraospedalieri, valutare il tasso autoptico definito come il rapporto tra il numero di autopsie effettuate sul numero totale dei decessi esaminati; - valutare l’accuratezza della diagnosi clinica formulata in un dipartimento di emergenza al momento del decesso, assumendo come standard di riferimento la diagnosi autoptica. Risultati I decessi intraospedalieri esaminati si riferiscono al periodo compreso tra giugno 2008 e dicembre 201l. Nello stesso periodo, gli accessi totali al Pronto Soccorso di Mestre ammontavano a 284.827 casi. I pazienti deceduti ammontavano a 223 soggetti e rappresentavano lo 0.078% degli accessi totali. Essi erano caratterizzati come segue: 28 casi nel 2008, 14 maschi e 14 femmine; 50 casi nel 2009, 24 maschi e 26 femmine; 83 casi nel 2010, 41 maschi e 41 femmine; 62 casi nel 2011, 27 maschi e 35 femmine. Nel periodo considerato (2008-2011), i pazienti deceduti erano 116 femmine e 107 maschi. L’età media era di 83,42 anni ±16,76 ds (casi di età compresa tra i 33 e i 106 anni) nelle femmine, mentre nei maschi era di 73 anni ± 3,53 ds (casi di età compresa tra i 22 e i 96 anni) e rappresentavano, rispettivamente, il 52,02% e il 47,98% dei decessi intraospedalieri. Tra le cause di decesso, quelle riconducili a patologia cardiaca, polmonare e cardiopolmonare rappresentavano più di 1/3 dei decessi intraospedalieri sia nel gruppo dei maschi che nel gruppo delle femmine. Dei 223 decessi, l’esame autoptico veniva richiesto nel 12,55% dei casi, ossia, in 28 pazienti (13 maschi e 15 femmine; l’età media era, rispettivamente, di 59 e di 70,2 anni). Dei 28 referti autoptici venivano analizzati solo 23 casi, ossia l’82,14% . Nella valutazione della concordanza tra la diagnosi di ‘dimissione’ e quella ‘autoptica’, i 23 casi erano così ripartiti: in 16 casi (69,57%) non vi era concordanza, mentre in 7 casi (30,43%) la concordanza era positiva cioè le due diagnosi erano uniformi e concordanti. Nell’analisi dei decessi intraospedalieri, l’età media dei maschi era significativamente inferiore rispetto alle femmine (59 vs 70,26 anni). Tra le cause di decesso, quelle per patologia cardiaca, polmonare e cardiopolmonare rappresentavano più di 1/3 dei decessi intraospedalieri sia nei maschi che nelle femmine. Le diagnosi ‘aspecifiche’ riconducibili ad arresto cardiocircolatorio e/o cardiorespiratorio e ad altro (exitus e cachessia) rappresentavano il 29,2% dei casi nei maschi e il 37,07% nelle femmine. Inoltre, in queste ultime, erano frequenti le cause riconducibili a sepsi ed a disfunzione multi organo. Le cause di decesso per eventi traumatici, in particolare, i traumatismi della strada erano più frequenti nei maschi rispetto alle femmine (15 vs 3 casi). Nell’analisi delle cause di decesso dei pazienti sottoposti ad autopsia, la totalità dei casi soffrivano di patologie severe, ‘life threatening’, a rapida evoluzione in exitus. Tra queste, erano prevalenti le patologie cardiache e vascolari sia addominali che cerebrali. Conclusioni Pur disponendo di ausili terapeutici e strumentali innovativi, questi non sempre contribuiscono a definire la vera causa di morte dei pazienti. Talora, la diagnosi clinica formulata dai sanitari è approssimativa e non corrispondente all’esame autoptico. L’accertamento anatomo-patologico rimane ancora di grande utilità nella pratica medica per stabilire la causa di morte, ma anche come fonte di conoscenza per migliorare i percorsi terapeutico- diagnostici, l’outcome dei malati nonché la pratica clinica dei sanitari.

SUMMARY A. OUTCOME OF PATIENTS PRESENTING THEMSELVES TO THE EMERGENCY DEPARTMENT WITH CHEST PAIN Background. Chest pain is one of the most common symptoms presented to the emergency department. It actually accounts for 5% - 8% of the total visits at the emergency room. Yellow triage tag is mainly assigned to patients with chest pain, when there are no alterations in their vital functions. ‘Short-stay observation unit’ is sometimes used for patients suffering from chest pain, which consists of additional monitoring in a specialised area of the emergency department, in order to exclude any case with high probability of acute coronary disease. The following table below shows the results of a retrospective study conducted at the Emergency Department of the “Ospedale dell’Angelo”, hospital located in the Venetian mainland of the Veneto region, in Italy. The study period was between 24th May 2010 and 23rd May 2011. The objective of the study was: -to investigate the outcome of patients with chest pain presenting to an emergency department; - to determine if triage is suitable for identifying chest pain with risk of acute coronary disease; -to evaluate if a ‘short-stay observation unit’ is useful and effective in case of undefined chest pain, that is potentially at risk of acute coronary disease. The results are as follows: 3,980 cases of chest pain were analyzed, which accounted for 4.71% of the 84,554 total visits to the emergency department during the period of the study. Among these, 2,449 patients (61.5%) were discharged, 714 (18.0%) were hospitalized, 4 (0.1%) died and 813 (20.4%) were transferred to short-stay intensive observation for further clinical bioumoral and electrocardiographic monitoring, and / or possible ergometric test. During primary triage, a red or yellow tag was assigned to 78.8% of cases of chest pain and a green or white tag was assigned to 21.2% of cases. The conclusive diagnosis revealed an acute coronary heart disease in 39.9% (285 patients) of the 714 patients who were directly admitted to the emergency room. Of the 813 patients submitted to short-stay observation unit, 73.4% (597 cases) were discharged, whereas 26.6% (216 cases) were admitted. Of 24.5% (199 cases) of the patients submitted to short-stay observation unit, 15.1% (30 cases) were discharged for chronic stable coronary disease and 84.9% (169 cases) were hospitalized for acute coronary syndrome. Finally, 454 patients with acute coronary disease were admitted to the emergency room and to short-stay observation unit, which accounted for 11.4% of the patients presenting themselves to the emergency department with chest pain. The death of four patients was due to complications of acute coronary disease. Furthermore, there was a prevalence of males over females among the patients admitted (64.4% males vs. 35.6% females). Triage proved to be sufficiently sensitive and specific in distinguishing the cases of chest pain for acute coronary disease at high risk from those at low risk. In fact, during primary triage the 78.8% of cases of chest pain were given a red or yellow tag (i.e., potentially life – threatening cases), and the remaining 21.2% of cases were given a green or white tag (therefore no life-threatening). The majority of patients who were given a green or white tag was sent home (84.3% vs. 55.4% of patients with a red or yellow tag), whereas a prevalent percentage of patients who were given a red or yellow tag was hospitalized (21%) or led to short-stay observation unit for further monitoring (23.5%). Patients used to stay in short-stay observation units slightly over 23 hours (23 hours and 32 minutes). Short-stay observation of chest pain at risk of acute coronary disease - in both patients with a yellow tag (high risk) and those with a green or white tag (low or intermediate risk) - turned out to be useful in reducing 'inappropriate' hospitalizations for nonspecific chest pain. In many cases it also allowed a rapid exclusion of acute coronary disease by means of telemetry monitoring, transthoracic echocardiography and ergometric tests, as well as cardiology specialist assessment. Conclusions Despite being a frequent symptom of presentation to the emergency department, acute chest pain and the risks attached require rapid clinical diagnosis and sometimes a diversified approach. If the cause of chest pain is not correctly identified, it could lead to fatal effects on the outcome of patients. Therefore, the intensive short-stay observation unit corresponds to the Chest Pain Unit, where patients with chest pain are further 'examined' to come to a definitive diagnosis, optimizing therapeutic - diagnostic treatments and the available resources. B. ANALYSIS OF IN-HOSPITAL DEATHS IN AN EMERGENCY DEPARTMENT Background. Dealing with critical health and life-threatening conditions is one of the main functions of the emergency department. During the initial assessment of the critically ill patient, assessment and stabilization processes have to be carried out in a very short period of time by following a step by step system, according to a scale of priorities (ABCD); shared and encoded diagnostic and therapeutic workups are often used to assess and stabilize the patient. Nevertheless, some life-threatening situations result in death. This is a retrospective study, conducted at the Emergency Department of the “Ospedale dell’Angelo” located in Mestre, in the north east of Italy. The purpose of the study was: - To analyze the most common causes of death in patients presenting themselves to an emergency department; - To evaluate the autopsy rate among in-hospital deaths, defined as the ratio between the number of autopsies carried out and the total number of deaths examined; - To evaluate the accuracy of clinical diagnosis made by an emergency department at the time of death, taking as a reference standard the autopsy diagnosis. In-hospital deaths were examined between June 2008 and December 2011. Results During the period between June 2008 and December 2011, the total admissions to the emergency room in Mestre amounted to 284,827 cases. Patients who died during this period amounted to 223 persons, representing 0.078% of the total admissions. They presented the following characteristics: 28 cases in 2008, 14 males and 14 females, 50 cases in 2009, 24 males and 26 females; 83 cases in 2010, 41 males and 41 females, 62 cases in 2011, 27 males and 35 females. During the period taken into account (2008-2011), patients who died were 116 females and 107 males, who accounted for, respectively, 52.02% and 47.98% of in-hospital deaths. The average age of females was 83.42 ± 16.76 years ds (cases between 33 and 106 years), whereas the average age of males was 73 years ± 3.53 ds (cases between 22 and 96 years). Among the causes of death, heart, pulmonary and cardiopulmonary diseases represented more than 1/3 of in-hospital deaths both for males and females. Of 223 deaths, an autopsy was required in 12.55% of cases, equal to 28 patients (13 males and 15 females; the average age was, respectively, 59 and 70.2 years). Of 28 autopsy reports, only 23 cases were analyzed, accounting for 82.14%. In evaluating the correlation between the ‘discharge’ diagnosis and the ‘autopsy’ diagnosis, the 23 cases were divided as follows: in 16 cases (69.57%) there was no correlation, whereas in 7 cases (30.43%), the correlation was positive since both diagnoses were equivalent. In the analysis of in-hospital deaths, the average age of death for males was significantly lower than for females (59 vs. 70.26 years). The 'nonspecific' diagnoses related to cardiac arrest and / or cardio respiratory arrest, among others, accounted for 29.2% of cases in males and 37.07% of cases in females. In addition, the most frequent causes of death in females were those related to sepsis and multi-organ dysfunction. The causes of death due to traumatic events, especially road accident traumas, were more frequent in males than in females (15 vs. 3 cases). The analysis of the causes of death in patients undergoing autopsy showed that the totality of cases suffered from severe life-threatening diseases, that rapidly led to death. The most common diseases were heart and vascular diseases, in particular abdominal and brain vascular diseases. Conclusions Innovative therapeutic and instrumental tools do not always contribute to determine the true cause of death in patients. At times, the clinical diagnosis made by physicians is not accurate and it does not reflect the post-mortem examination. The anatomo-pathological assessment is not only extremely useful in medical practice in order to determine the cause of death, but it is also a source of knowledge to improve therapeutic-diagnostic workups, the outcome of the patients, as well as the clinical practice of physicians.
